The Indian Health Service was built as a primary health care system for rural, not urban area.

Over the years American Indians have migrated either by choice or through relocations to urban areas, and so, yeah, the census has shown that there are more American Indians, Alaska Natives who self-identify, as such, living in urban areas, and that’s a challenge for us because the Indian Health Service was primarily built as a primary care healthcare system in rural areas, so all of the urban American Indians have challenges accessing healthcare.
